Shift
Sunday through Friday, 11pm - 7am
What We Offer
- Provides continuous supervision and case management services to assigned consumers on caseload
- Assists consumers with their activities of daily living skills, budgeting, and linkage to applicable benefits
- Provides consumers with medication education and observes their self-administration of medication
- Must adhere to the agency’s driving policy of transporting consumers in a safe and responsible manner
- Provides input to treatment and service plans as well as their implementation

Must be able to document all relevant information to the consumers' electronic chart records
- BSW in social work, BA/BS in psychology, criminal justice, sociology, or a related field
- BA in an unrelated field with two years of previous experience
- AA in a related field with two years of previous experience
- Previous experience desired
