Job Description
We are currently seeking a full-time Nursing (Care) Team Lead to join our team at The Journey Club within the beautiful lakeside community of Westman Village in Mahogany.

We are looking for a compassionate, highly-trained, and empathetic Nursing (Care) Team Lead, the Butterfly Model of Care at The Journey Club is designed to look and feel just like a real home for seniors’ experiencing dementia.
The Nursing (Care) Team Lead is responsible for the supervision of the LPNs and Healthcare Aides and is accountable for the delivery of patient care utilizing the nursing process within the standards, policies, procedures and guidelines of Journey Club. The Team Lead must be able to function with limited supervision and partner as required with the Director of Care
A day in the life of a Nursing Care Team Lead:
- Delivers resident-focused care within full scope of practice of a LPN, in a home-like environment, respecting the individuality and dignity of each resident/ family and co-workers.
- Responsible for regular and ongoing communication to team members to ensure they are well informed about current activities, resident status, changes in assignments and daily routines
- Assisting with the coordination of students for training, responsibility for training, orientation, competency assessment with the Director of Care
- Care Evaluations for Marketing on Independent Residents.
- Tour of potential PAL Residents.
- Responsible for supervising and providing leadership to LPN and HCA teams while on shift to ensure AHS and Asura Health standards for PAL and private care are met.
- Partner with Director of Care to ensure effective employee relations, conflict resolution and action plans as required.
- Participation in weekly meetings including but not limited to Director of Care, Executive Director, Recreation, Physicians and Pharmacy.
- Maintains a professional standard of discretion and confidentiality.
- Provides leadership or assistance as required in emergency situations.
- Maintains confidentiality regarding patient and business functions of the facility.
- Partner with Director of Care to ensure all requirements are met, audits completed and effective and thorough follow up.
- Audit processes, suite cleaning, resident care, Risk Assessment etc.
- Practice to their full range of competence within applicable legislation, regulations, bylaws and employer policies.
- Responsible for Multi-disciplinary Rounds, this includes Physical, pharmacy and nursing round. This will be at the discretion of Director of Care.
- This position is two (2) days administration in the office and three (3) days on the floor.
- Must be available to work 1 evening and 1 night shift per month.
- Must be available to work evenings and weekends, as required, for emergency calls.
- To be on-call as required.
- Other duties as required.
What you bring to the table:
- Licensed Practical Nurse, experience in gerontology preferred
- Current membership in good standing with CLPNA
- CPR/BCLS for healthcare provider required
- Clear police information check within 6 months
- Demonstrated skills in leadership, communication, teamwork, concern resolution process and customer service
- Strong organizational and computer skills are required in this position
- Ability to problem solve through critical thinking and self-education
- Enthusiastic attitude and energy to provide excellent care and service on behalf of the company
- Awareness of accident and injury prevention, hazard identification and control; adherence to safe work practices and procedures is essential in this position
- Demonstrated knowledge and commitment to person-centered care
- Demonstrated commitment to the mission, vision and values of Advantage Assist Group Inc. is essential
- Physically and mentally fit to perform the duties of the position (may be required to lift, move, push, or pull objects weighing up to 75 pounds)
- Ability to work on his/her feet for a complete shift while constantly bending, stretching, walking, lifting or moving things.
