Personal Care Assistant

Job Summary
We are seeking a compassionate and dedicated Personal Care Assistant to provide essential support to individuals in need of assistance with daily living activities. The ideal candidate will have experience in home community care, memory care, or similar environments, and will be committed to enhancing the quality of life for our clients. This role requires a strong focus on patient observation and the ability to implement care plans effectively. The client has early stage dementia, and needs to be monitoring at all times.

Responsibilities

• Assist clients with personal hygiene, grooming, and bathing as needed.
• Help with mobility and transfers to ensure client safety and comfort.
• Monitor vital signs and report any changes in health status to the supervising nurse or healthcare provider.
• Implement individualized care plans tailored to each client's needs.
• Provide companionship and engage clients in meaningful activities to promote mental well-being.
• Assist with meal preparation and feeding as required.
• Maintain a clean and safe environment for clients, ensuring all areas are tidy and organized.
• Document daily activities and observations accurately in client records.
• Collaborate with healthcare professionals, including nurses and direct support staff, to ensure comprehensive care.

Qualifications

• Previous experience as a Personal Care Assistant or in a similar role is preferred.
• Knowledge of home health care practices, including memory care techniques.
• Ability to observe patients effectively and report changes in condition promptly.
• Familiarity with vital sign monitoring is advantageous.
• Strong interpersonal skills with the ability to build rapport with clients and their families.
• Compassionate nature with a genuine desire to help others maintain their independence.
• High school diploma or equivalent; relevant certifications in care giving or nursing assistance are a plus.

Pay: $14.00 - $15.00 per hour
