POSITION: 10pm-6am, rotating schedule

Job Summary

The Licensed Staff Nurse is responsible for the nursing care of the facility residents. The Licensed Staff Nurse communicates with off going and arriving shifts of personnel to assure the continuum of quality of care. The Licensed Staff Nurse makes rounds, evaluates the physical, emotional and social needs of the residents to ensure the staff assigned provides intervention as indicated. Supervises the care provided by the Nursing Assistants assigned to the unit, providing intervention, and education as indicated. Prepares and administers medications and treatments according to procedures. Consults with physicians, receives, records and transcribes orders as required. Makes rounds with physicians when necessary. Reviews resident’s care plans and assures these plans are followed. Documents on records as per policy and procedures of the facility. Participates in the evaluation of the employees working under his/her supervision. Participates in staff meetings and in-services as required. The Registered Nurse, Licensed Practical Nurse or Licensed Vocational Nurse is responsible to the Director of Nursing and/or the Assistant Director of Nursing.

Education

The Licensed Staff Nurse must be a graduate of an accredited school of nursing currently registered with the state agency for nursing licensure and hold a valid nursing license in the state in which employed.

Qualifications

1. Demonstrates leadership and managerial ability.

2. Has the ability to organize, plan, and supervise the Nursing Assistants assigned to work a designated unit and shift.

3. Maintains effective interpersonal relationships and works well with the general public.

4. Is of good emotional, mental and physical health, having sound judgment and high professional standards.

5. Must be willing and capable of providing emergency care as needed for any resident in the facility.

6. Is courteous in working with residents, families and employees.

7. Maintains a neat, well-groomed and professional appearance at all times.

8. Has a desire to work with the geriatric clientele as well as residents of any age and their current and prospective needs.

9. Must be willing to be in the facility ready to begin assigned duties on time.

10. Must be willing to be in the facility during weekdays as well as weekend hours.

11. Must be willing to participate in the on call schedule.

12. Must be willing to work preferred shifts as scheduled and assist on other shifts as needed to assure adequate staffing to meet the needs of each resident.

13. Must demonstrate the ability to function as a cooperative team member with all disciplines in the facility.

14. Must demonstrate the ability to follow and supervise assigned staff to assure the facilities policies and procedures are upheld.

15. Must demonstrate the ability to manage and supervise the assigned unit in a productive, timely manner.

16. Must have a pre-employment and annual Tuberculosis screening examination as required by the state in which employed.

17. Must maintain a current healthcare provider cardio-pulmonary resuscitation certification.

Responsibilities

The Licensed Staff works with the Director of Nursing to assure the following responsibilities are accomplished. This list is only partial and should not be interpreted as a comprehensive inventory of all duties, tasks and responsibilities.

1 Receives report from the off going shift and gives report to on coming shift.

2 Reviews communication systems to be knowledgeable of any unusual occurrences during the previous shifts.

3 Assigns nursing assistants and other designated personnel assuring each resident’s needs will be met during the assigned shift.

4 Gives report to Nursing Assistants.

5 Makes rounds of the assigned unit frequently during the shift providing intervention as needed. During the entire shift, will be attune to needs of the unit while doing other nursing tasks.

6 Prepares and administers medications as ordered using the 5 rights method. Reorders medications as needed in preparation for future shifts. Assesses and documents medication issues as per policy.

7 Provides special procedures as ordered.

8 Completes treatments as ordered. Assesses and documents accordingly. Contacts the physician for any treatment issues as per policy.

9 Supervises the meal services for each resident on the unit. Works with the staff to assure each resident is provided adequate nutrition for each meal.

10 Monitors residents for risk or complications as a result of the residents’ medical conditions. (i.e., skin breakdown, incontinence, contractures, reduced mobility, reduced involvement in bathing, dressing, or eating, weight loss, behaviors due to change in lifestyle, etc.)

11 Observes and monitors the Nursing assistants to assure that resident’s needs are met and nurse aides duties are performed.

12 Document on nurse’s notes as per policy. Assures there is accurate and timely documentation of any unusual observation, incident, or situation.

13 Reviews and follows resident’s plan of care.

14 Admits residents as required.

15 Assists in the ordering and reviewing of results of laboratory tests as indicated. Notifies physician of results.

16 Notifies the physician of unusual findings as indicated.

17 Assists with auditing charts.

18 Counts required medications with oncoming and off going shifts.

19 Assists in keeping other departments informed of resident’s change of condition as indicated.

20 Assists in providing in-services for assigned staff.

21 Keeps nurse’s station, medication room and other work spaces clean and orderly.

22 Participates with month end physician order reviews.

23 Makes rounds toward end of shift to assure area is in order for the oncoming shift.

24 Reviews and assures there is adequate staffing for the oncoming shift. Replaces known absences in a timely manner.

25 Notifies the Resident and/or Resident Representative of changes in condition or changes to the plan of care.

26 Prepares incident/accident reports as needed.

27 Reports immediately to supervisor, administrator and local law enforcement situations involving abuse, neglect, or suspicion of a crime.

28 Adheres to the Code of Conduct, Compliance Program, HIPAA Standard, and Computer Usage Agreement.

29 Performs any other tasks as assigned by the Director of Nursing or Assistant Director of Nursing.

2. Completes educational requirements per policy.

3. Maintains regulatory requirements.

4. Occasional travel/overnights required for training, professional meetings, emergencies, and evacuations.

5. Reports to work on time and as scheduled.

6. Wears identification while on duty.

7. Must work throughout all areas of the facility.

8. Completes work in designated time frames.
